Members and Volunteers Consent to Liability Disclaimer

By agreeing to become a member of SOAR Family Support Inc, you provide consent to the following

1. Acknowledgment of Risk.  I acknowledge my participation as a Member/Volunteer with SOAR Family Support Inc. (“SOAR”), a 501(c)(3) nonprofit organization, is voluntary and may involve certain risks, including but not limited to physical injury, property damage, or other unforeseen risks.
2. Assumption of Risk. I understand and accept these risks and agree to assume full responsibility for any injuries, damages, or losses that may occur as a result of my participation in Member/Member/Volunteer activities with SOAR.
3. Release and Waiver of Liability.  I hereby release, waive, and hold harmless SOAR, its directors, officers, employees, agents, and affiliates from any and all liability, claims, demands, actions, or causes of action related to any loss, damage, or injury sustained in connection with my Member/Member/Volunteer activities, whether caused by negligence or otherwise.
4. Indemnification. I agree to indemnify and hold harmless SOAR from any claims, damages, losses, or liabilities, including legal fees, arising from my actions or conduct while Member/Member/Volunteering.
5. Medical Treatment. I consent to receive medical treatment in the event of an injury or illness while Member/Member/Volunteering. I understand that SOAR does not provide health insurance, and I am responsible for any medical costs incurred.
6. Confidentiality.  I acknowledge that during my Member/Member/Volunteer service, I may have access to confidential or proprietary information regarding SOAR, its operations, donors, beneficiaries, or other Member/Member/Volunteers. I agree to maintain the confidentiality of such information and not to disclose or use it for any purpose other than my Member/Member/Volunteer activities. This obligation remains in effect even after my Member/Member/Volunteer service has ended.
7. No Employment Relationship. I acknowledge that my Member/Member/Volunteer service does not create an employment relationship between myself and SOAR, and I am not entitled to compensation, benefits, or legal protections afforded to employees.
8. Photographic Release. I grant permission for SOAR to use photographs or video recordings taken of me during Member/Member/Volunteer activities for promotional and marketing purposes without compensation.
9. Governing Law. This agreement shall be governed by the laws of the state of Georgia, and any disputes arising from this agreement shall be resolved in the appropriate courts of that jurisdiction.
10. Acknowledgment and Agreement. I have carefully read and fully understand this liability disclaimer. By signing below, I voluntarily agree to its terms and conditions.
